ABSTRACT:     The accumulation of sodium in the body leads to the development of hypervolemia, followed by an increase in cardiac output. At the same time, the volume of extracellular fluid increases, the sodium content in the vascular wall increases, which leads to its swelling and increased sensitivity to the pressor effects of vasoconstrictor hormones (angiotensin, vasopressin, vasoconstrictor hormones of the endothelium). The reason for the retention of sodium and water in the body in renal pathology is a decrease in organ perfusion and a violation of the "processing" of sodium in the nephron - a decrease in its filtration and increased electrolyte reabsorption as it passes through the renal tubules, which is primarily associated with a decrease in the mass of active nephrons. From 2020 to 2022, 183 patients undergoing programmed hemodialysis were selected at the Republican Specialized Scientific and Practical Medical Center for Nephrology and Kidney Transplantation (RSNPMCN and KT), of which 43 patients were after heterotypic kidney transplantation from a living related donor. There were 148 men and 35 women. These patients were examined for the presence of arterial hypertension (AH). Hypertension syndrome (systolic blood pressure > 140 and/or diastolic blood pressure > 90 mmri. art.) Thus, a positive association in patients with CGN, in whom blood pressure varied from 140/90 to 160/110 mmHg, was revealed for the type 1 receptor gene to angiotensin II -1166A/C AGTR1 genotype C/C, and resistance for the type 1 receptor gene to angiotensin II -1166A/With AGTR1 genotype A/C. It should also be noted that for a group of patients before and after kidney transplantation from a living related donor, these data were ideal.
