Retrospective analysis of ambulatory management of patients with chronic heart failure

ABSTRACT: To establish the possibility of strictly monitoring CHF patients after inpatient
treatment and to determine the effectiveness of their observation in a polyclinic at the place of
residence using the example of two districts of the Samarkand region.
The patients were divided into 2 groups. The first group consisted of patients with CHF belonging
to the 1st family polyclinic of the city of Samarkand, the 2nd group consisted of CHF patients of
the district of the Samarkand region. The study included 289 patients with CHF in the 1st group
and 728 patients in the 2nd group, who, after discharge, were on outpatient observation at the
place of registration in 1 family polyclinic in Samarkand.
Currently, patients with CHF in the city of Samarkand and the Ishtykhan region are practically
not actively observed by doctors at the outpatient clinics at their place of residence. Doctors did
not go to house-to-door rounds, did not actively call-in patients, and patients had to call an
ambulance if their condition worsened. Hence the high mortality rate of patients with chronic
heart failure: in the Samarkand polyclinic - 271, in the Ishtykhan district - 572 in 2020.
